When treating an evacuee, My Canadian Pharmacy will ask to provide the evacuee’s first name, second name, date of birth, zip code, and sex. If you come across with the disaster previously you need to provide the following info:
- quantity and day supply;
- the pharmacy that filled the refill;
- the physician who prescribed the script;
- medication information, such as indication and dosage, administration, and interactions.
